RN Hospice Admission

Role Overview: The Hospice Admission RN is a licensed nurse responsible for educating patients and families about hospice services and palliative care, ensuring informed decisions for hospice admission. Key Responsibilities: Conduct comprehensive assessments review medical histories and medication profiles coordinate resources develop individualized care plans obtain physician orders educate families on care processes and financial responsibilities Collaborate with interdisciplinary teams to determine care goals establish visit schedules provide emotional support during the transition to hospice Ensure compliance with healthcare standards and policies Qualifications & Skills: Minimum of 2 years acute care and hospice experience preferred Valid RN license, BLS certification, and valid driver’s license with insurance are required Strong assessment, communication, and teamwork skills are essential Knowledge of hospice eligibility criteria, medication management, and patient-centered care practices is necessary Additional Notes: Must uphold professional nursing standards, support service excellence, and demonstrate compassion in patient care. #J-18808-Ljbffr Plan A Health
