Join us in making a real difference in the health of our community.Job SummaryPosition title: Director of Integration, Care ManagementReporting to: Senior Vice President, OperationsThe Director of Integration, Care Management will lead the planning and execution of integration initiatives across Essen Health Care's Care Management Division. This role is responsible for managing complex projects that bring together clinical operations, partnerships, technology, and workflows to ensure successful onboarding of new partners, programs, and service lines.The ideal candidate is a highly organized healthcare leader with strong project management experience, operational insight, and the ability to drive cross-functional collaboration across clinical, IT, compliance, finance, and operations teams.ResponsibilitiesIntegration & Project ManagementLead end-to-end integration projects for new Care Management partners, programs, and operational initiativesDevelop detailed project plans, timelines, milestones, and deliverablesEnsure successful implementation of workflows, staffing models, systems, and reporting structuresIdentify risks, dependencies, and mitigation strategies to keep projects on trackServe as the central point of coordination between internal departments and external partnersOperational ExecutionTranslate strategic goals into executable operational plansStandardize onboarding and integration processes for new partners and care management programsEnsure alignment between clinical operations, compliance, and performance expectationsMonitor post-integration performance and address operational gapsCross-Functional CollaborationPartner with Clinical Leadership, IT, Compliance, Finance, HR, and Operations to ensure seamless integrationFacilitate regular project meetings, updates, and stakeholder communicationsDrive accountability across workstreams and ensure deadlines are metProcess Improvement & OptimizationDevelop scalable integration frameworks and best practicesContinuously improve workflows for care management delivery and partner engagementLeverage data and performance metrics to inform decision-makingReporting & GovernanceProvide regular project status reports to executive leadershipTrack KPIs related to integration success, operational readiness, and partner performanceSupport regulatory and compliance readiness for all integration initiativesQualificationsRequired Qualifications5–7+ years of experience in healthcare operations, project management, or care management integrationProven experience leading complex, cross-functional projects in healthcare environmentsStrong understanding of care management, population health, or value-based care modelsExperience working with Medicaid managed care and regulatory frameworks preferredExcellent organizational, communication, and stakeholder management skillsBachelor's degree required; Master's degree (MBA, MHA, MPH) preferredPMP or formal project management training strongly preferredPreferred QualificationsExperience integrating new healthcare partners or service linesBackground in scaling healthcare operations across multiple sitesFamiliarity with clinical workflows, EHRs, and care management platformsKnowledge of NYC/Bronx healthcare marketCore CompetenciesProject Leadership & ExecutionStrategic Planning & Operational IntegrationChange ManagementProcess ImprovementCross-Functional CollaborationData-Driven Decision MakingEqual Opportunity EmployerEssen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population.
