We're currently looking for a Case Manager Registered Nurse for a Hospital contract.Job Title: Case Manager Registered Nurse Location: Randallstown, MD Setting: Hospital Pay Range: 1856.35 - 1949.17 Schedule: 08:00 - 17:30 Contract Length: 13 weeks Candidate Type: Traveler Requirements: BLS; Active Maryland RN license; Minimum 2 years of hospital inpatient RN Case Management/discharge planning experience; Med-Surg and/or Critical Care discharge planning experience; Strong inpatient discharge planning experience; Experience coordinating discharge needs, resources and transitions of care; Must be able to work independently after approximately 3 days of orientation.Responsibilities:Manage discharge planning and patient transitions in the Emergency Department and medical inpatient units.Support adult and geriatric patients, ensuring safe and effective care coordination.Collaborate with Social Work to coordinate patient discharges and transitions of care.Support approximately 1.5-2 units during crisis staffing in partnership with Case Management and Social Work.Ensure compliance with hospital policies and clinical protocols using Cerner charting system.Why Choose VyTalent Solutions?
