As a Personal Care RN, here's a big-picture view of what you'll do:

Conduct initial admissions, comprehensive assessments, and routine reassessments of patients receiving personal care services
Develop, implement, and update individualized Plans of Care in collaboration with physicians, patients, and caregivers
Perform supervisory visits with home care aides to ensure care is delivered in accordance with the Plan of Care and agency standards
Provide ongoing clinical oversight, guidance, and education to personal care aides and caregivers
Evaluate aide competency and document supervisory visits per regulatory requirements
Monitor patient condition and promptly communicate changes to physicians and interdisciplinary team members
Provide skilled nursing services as needed and ensure all care aligns with physician orders and state regulations
Educate patients and families on disease processes, safety, and care needs
Ensure accurate, timely, and compliant documentation of all visits, care plans, and communications
Coordinate services and collaborate with internal teams to ensure continuity and quality of care
Participate in case conferences, quality assurance activities, and performance improvement initiatives
Assist with care transitions and discharge planning as appropriate

A few must-haves for PCSS Registered Nurses:

Graduate of an accredited nursing program and active RN license in Virginia
Minimum of 2 years of RN experience or completion of a preceptor program
Active CPR certification (adult, child, and infant)
Knowledge of personal care services regulations and supervisory requirements preferred
Strong clinical judgment, communication, and organizational skills
Ability to manage multiple patients and priorities independently
Valid Driver's License and reliable transportation
