Registered Nurse Case Manager - Hospice

Job Title: Registered Nurse Case Manager - Hospice Location: Cheswick, PA, USA (Additional Locations) Employee Type: Employee. Working Hours: 40 per week. What You Will Do Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning. Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for. Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation. Work independently, but never alone. Travel to patient homes within the assigned territory. Two years of RN experience with the clinical expertise to deliver quality patient care; hospice experience preferred. Current driver’s license and ability to spend approximately 20% of your day driving to and from patient locations. Job Eligibility: This position is eligible for a $10,000 first‑year success bonus to reward the accomplishment of key milestones during your first year.
