Care Management Specialist PRN - Rehab & Discharge Planning
Sheltering Arms Institute
A leading rehabilitation hospital is seeking a Care Management Specialist PRN to provide patient care and coordinate team-delivered services. The specialist will conduct psychosocial screenings, support patients and families, and ensure optimal discharge planning.
