Case Manager

Under general supervision is responsible for providing case management and/or treatment according to established policies and procedures. The individual will assist in creating and fostering an environment conducive to the treatment, operational and administrative goals and mission of Mon. Day. Summary of Job Duties Essential Duties and Responsibilities. Case Management. Sets appointments, completes transportation requests, visitor lists, furlough verifications and obtains court orders. Determines if residents have their social security card, birth certificate and state ID/driver license. Assists with obtaining above mentioned documentation as needed Investigates and set up court dates for residents with warrants. Assists in the facilitation with classes to support when clinical staff are out of the office Provides transportation as necessary.
