About This Role The Care Manager (RN) develops, assesses, and facilitates complex care management activities for members with physical needs, focusing on personalized care plans and education for members and their families. Highlights Location: Remote, GA Schedule: Full Time, Day shift Residency: Must reside in Georgia What You'll Do Evaluate the needs of the member and recommend a plan for the best outcome. Develop ongoing care plans and collaborate with providers to address unmet needs. Identify problems/barriers to care and provide appropriate interventions. Coordinate between the member and care provider team for person-centered care. Provide ongoing follow-up and monitoring of member status and care plan goals. Provide resource support for local services based on assessments. Facilitate care management and collaborate with providers for timely access to care. Perform telephonic, digital, home, and/or other site outreach to assess member needs. Collect, document, and maintain member information for compliance. Provide education to members and their families on care options and healthcare benefits. Requirements Education: Degree from an Accredited School of Nursing or Bachelor's degree in Nursing required. Experience: 2 – 4 years of related experience required. Licenses & Certifications: RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
