• Schedule: Monday–Friday | 8:00 AM–5:00 PM
• Hutchinson, KS and surrounding areas

Hospice Social Workers offer care and assistance to individuals in the final stage of life, as well as their families and close ones. Their purpose is to provide emotional and practical support, with the goal of enhancing the well-being of those confronting end-of-life challenges. Hospice social workers collaborate closely with healthcare teams to organize treatments, services, and available resources, ultimately aiming to enhance a patient's overall quality of life.

Requirements

• Master’s in Social Work (MSW) or LCSW required
• Hospice experience preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Professional, dependable, and organized

Responsibilities

• Provide counseling and support to patients and families in the home
• Assist with advance directives and community resources
• Participate in the hospice interdisciplinary team (IDG)
• Coordinate referrals and additional support services
• Maintain positive relationships with patients, families, staff, and referral partners
