Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment centered on healing and wellness. Nursing practices are based on evidence and current research, as well as hospital policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit. This includes provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is essential. The RN provides patient and family-centered education. Care coordination and individualized treatment planning are also responsibilities. Assessment of patient needs is a critical duty. The RN performs and documents a comprehensive assessment in accordance with the scope of practice and policies. Consideration of the patient’s culture, age, and developmental level is necessary. Identifying patient risk factors and presenting complaints is essential. The RN obtains patient health information from family or others as necessary. Objective and subjective data collection is performed. Verification of initial admission assessment, if obtained by other team members, is required. Development of a problem list and prioritizing patient needs is crucial. Action design and interventions relevant to the patient’s culture and age are developed. Desired outcomes are determined in coordination with the patient and family. The RN fully implements patient care therapies and procedures. This is done with empowerment and clinical autonomy in accordance with established policies and guidelines. The RN explains procedures, treatments, and processes to patients and families. Initiating actions to alleviate patient stress and discomfort is part of the role. The RN recognizes, responds to, and communicates changes in patient condition appropriately. Ongoing assessment and response to procedures and therapies using the nursing process is conducted. Tasks are delegated appropriately, and team members are overseen in the performance of patient care activities. The RN monitors the accuracy and completeness of delegated patient care. Identifying and addressing educational needs of patients is key. Facilitating the patient discharge process involves executing discharge orders in a timely manner. The RN provides input into patient/nurse assignments considering staffing and patient acuity. Active participation in unit-based activities in support of organizational values is expected. Promoting a positive group dynamic and healing environment through words and actions is important. The RN assures nursing excellence in accordance with accepted standards and evidence-based practice. Participation in department and unit quality improvement activities and core measures is encouraged. A holistic perspective regarding patient care is utilized.
