Social Worker

Heartland Hospice, part of ProMedica, seeks a compassionate Social Worker to support patients with life-limiting illnesses and their families. The BSW Social Worker conducts psychosocial assessments, develops care plans, and provides counseling, grief support, and resource coordination in home and facility settings. This role collaborates closely with an interdisciplinary hospice team, including nurses, chaplains, and physicians, to promote comfort, dignity, and patient-centered goals. The position involves advocacy, crisis intervention, documentation, and community referrals while upholding a mission-driven culture of respect, teamwork, and professional growth.
