• Collaborate with physicians, advanced practice providers, and interdisciplinary teams to create, implement, and assess personalized care plans.
• Initiate and oversee IV therapy, wound care, post-operative care, and telemetry or cardiac monitoring as necessary.
• Ensure precise and timely documentation in the electronic medical record.
• Educate patients and their families on diagnoses, medications, discharge instructions, and ongoing care requirements.
• Support safe transitions from hospital to home or other care environments.
• Delegate appropriate tasks to LPNs and CNAs while upholding infection control and safety standards.
• Engage in quality and performance improvement initiatives.
