Position Summary
We are seeking a highly motivated, Personal Health Nurse to join our team. This full-time, hybrid role combines in-person visits at our client sites with remote care coordination and telehealth support.
Key Responsibilities
Provide direct nursing care, health assessments, and chronic disease management to clients.
Conduct in-home or onsite visits and deliver telehealth telephonic support as needed.
Develop individualized care plans, coordinate services, and monitor progress.
Educate clients and families on preventive health, medication adherence, and self-management.
Collaborate with interdisciplinary teams, community partners, and other healthcare providers.
Maintain accurate electronic health records and comply with HIPAA and other regulatory standards.
Qualifications
Current RN license (State of DE) in good standing.
Bachelor’s degree in Nursing preferred.
Bilingual fluency in English and Spanish is a plus!
Minimum 2 years of nursing experience, preferably in community health, primary care, or care coordination.
Strong communication, cultural competence, and organizational skills.
Proficiency with electronic health records and telehealth platforms.
Schedule & Work Environment
Full-time (40 hours per week), Monday–Friday.
