Waiver Case Management Specialist: OIDD - Central Office

Responsibilities Conduct a monthly review of assigned Waiver enrolled individuals to ensure at least two waiver services are being utilized on a monthly basis. Complete monitoring of the Residential Needs List and provide case management consultation on challenging cases by reviewing case history, researching potential resources to meet needs, and working with the case manager to implement supports and meet identified needs. Monitor and address service authorization issues for waiver participants statewide. Make necessary corrections when the case manager is not immediately available. Provide case management services for assigned cases, document activities as required, and provide requested updates to supervisor. Serve as the Appointed Representative to assist assigned individuals with applying for Social Security (SSI/SSDI) benefits. Perform other case management functions as assigned and meet division expectations. Qualifications A bachelor’s degree from an accredited college or university and at least one year of experience working with the target population, including experience in the OIDD service delivery system. Knowledge of inter‑agency case management principles and practices and familiarity with populations served by OIDD (individuals with intellectual disabilities, related disabilities, brain injury, spinal cord injury and similar disabilities) as well as available services/supports. Knowledge of Medicaid State Plan and OIDD operated HCB Waiver services is required. Ability to interpret and apply policies and procedures. Excellent verbal and written communication skills, including the ability to communicate effectively. Computer literacy. Ability to exercise judgment and discretion and establish and maintain effective working relationships.
