Patient Liaison - Medical Practice

What You’ll Do Serve as the primary point of contact for a designated medical provider, managing day-to-day coordination and communication. Ensure providers are fully prepared for their appointments with organized schedules and up-to-date patient information. Deliver exceptional client service by educating patients, managing follow-ups, and facilitating post-visit needs like labs, supplements, and referrals. Communicate with warmth, clarity, and attentiveness to ensure seamless continuity of care. Coordinate with internal and external providers, retrieve medical records, and help relay wellness recommendations in line with our whole-person approach. Support team operations by stepping into clinical or administrative roles when qualified and needed. Requirements Required Qualifications Bachelor’s degree (B.S. or B.A.) or Licensed Practical Nurse (LPN) (required) One years’ medical office experience (required); will consider other combinations of education and experience Experience with medical terminology and working in a clinic or hospital setting (required) Completion of coursework in medical terminology and medical records (preferred) Strong computer and word processing skills (required) EMR and scheduling experience (preferred) Why You’ll Love Working at PALM Health Join a mission-driven organization where well-being is a way of life.
