RN Case Manager: Discharge & Care Coordination

Saint Francis Hospital

Saint Francis Hospital, Inc. seeks a Case Manager (RN) to provide case management services, identify discharge needs, and coordinate discharge planning with the patient and care team. The role follows a Monday–Friday, 8:00 AM – 4:30 PM schedule and aims to develop safe discharge plans that minimize readmissions. You will work with physicians, nurses, and social workers, plan care, ensure continuity, and obtain payer authorizations for post-acute services. #J-18808-Ljbffr
