TITLE : Clinic RN II DEPARTMENT: Health
CLASSIFICATION: Non-Exempt
DUTIES & RESPONSIBILITIES :
- Work closely with clinical staff and functions as a collaborative member of the health care team.
- Monitor and perform accurate and appropriate assessments for all assigned adult and pediatric patients, identifying and collecting data concerning the health status and clinical picture of each patient.
- Monitor, anticipate and respond to emergency situations. Initiate appropriate measures in emergency situations when the primary care provider is not immediately available. Start IV if ordered by primary care provider.
- Responsibly provide appropriate documentation of all observed patient information and patient care in the patient's clinical record. Maintain knowledge of EHR.
- Assist with implementation of the medical plan of treatment and administration of therapeutic measures as prescribed by the Physician/Provider such as medications, treatments, dressing changes, collection of specimens and catheterizations. Assisting with implementing and evaluating individual patient care to include making referral appointments to outside providers.
- Perform duties to meet meaningful use requirements. Maintain knowledge of CMS requirements and best practices.
- Administer medications to patients as assigned by Physicians/Providers, observe patients for reaction to drugs given and reports adverse reactions or condition changes and all medication errors immediately.
- Assist in person or via phone with assessment, planning, providing, evaluating and documenting instructions and teaching to patients and families in preventive, curative and rehabilitative measures. (Telephone triage)
- Assist physician/provider (including visiting providers) with examinations/procedures, adhering to clean/sterile techniques as needed.
- Serve as the primary lead for the coordination of Durable Medical Equipment (DME), including assessment of needs, selection of appropriate equipment, and management of insurance authorizations.
- Manage care transitions for patients returning home from hospitals, nursing facilities, or acute care settings. Act as the liaison between discharge planners, providers, and community resources to ensure a seamless transition.
- Coordinate the delivery, setup, and patient education regarding DME to ensure patients and caregivers understand proper use, safety and maintenance.
- Conduct follow-up communications (phone or in-person) to troubleshoot DME issues, assess compliance, and ensure patient safety post-discharge.
- Perform case management duties as assigned, including developing and implementing care plans for patients with chronic or complex conditions to improve outcomes.
- Share responsibility for sustaining adequate supplies and equipment as well as general upkeep of the clinic and in preserving a safe environment.
- Shares responsibility for cost-effective utilization of supplies and equipment.
- Collect and label chemistry, microbiology and hematology lab specimens and process all labs in accordance with lab policies and procedure.
- Maintain knowledge of and adheres to current Infection Control policies and standards of practice. (Standard precautions, Respiratory Precautions, Isolation requirements, etc.)
- Maintain and promote professional growth for self and other staff to include staff/community training in first aid/CPR.
- Maintain patient confidentiality and adhere to "Patient's Rights" policy. Respect patient privacy and promote patient's total well-being. Adhere to and promote professional, ethical and legal standards of nursing care and policy and procedures of the Tribal Clinic.

- Other duties as delegated by Clinical Director and Clinic Nurse Manager.
REQUIREMENTS:
- Associate of Science Degree in nursing required. Bachelor of Science in nursing preferred
- Must be a Registered Nurse in the State of Iowa with 4 years of professional nursing experience
- Prefer 5 years of ambulatory clinic experience
- Require competency in IV therapy and central line management (PICC line management)
- Must be CPR certified, Instructor preferred
- Preferred candidates must have experience in one or more of the following areas of nursing: Diabetes care, chronic disease management/home health, Mental Health/Behavioral Health and Substance abuse nursing, and/or Pediatric nursing care
- Must possess basic knowledge of wide variety of medical disorders, pharmaceuticals and ability to recognize and react to emergency situations
- Must possess the ability to work well with a diverse staff. Great interpersonal and communication skills required. Must be able to adapt to change in a fast-paced health care setting.
- Electronic health record experience required
- Must be willing to obtain Advanced Cardiac Life Support certification within 12 months of hire date, Instructor preferred.
- Must submit to and successfully complete background investigation.
SUPERVISION :
- Supervised by the Clinic Nurse Manager.
SALARY LEVEL :
- Salary Range: $77,147.20 - $105,497.60
