Job Description

Administer skilled nursing care for clients of all ages in their place of residence.
Assess home health and hospice patients to identify physical, psychosocial, and environmental needs.
Implement, develop, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
Provide education to patients and their families regarding care, diet, medications, and home safety.
Supervise and provide clinical direction to home health aides to ensure service quality.
Monitor assigned cases to ensure compliance with third-party payer requirements.
Perform on-call responsibilities and provide on-call services as needed.

Critical Information

On-call required if requested; on-call needs are subject to change based on staff and branch needs.

Education/Licenses Needed

Current state license as a Registered Nurse.
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
CHPN certification preferred.
Proof of current CPR certification.

Benefits:
