Licensed Practical Nurse LPN

UnitedHealth Group

Commonwealth Home Health, a part of LHC Group under the Optum family, seeks an LPN to provide in-home patient care. You will work within a care plan, and collaborate with the health care team to support families and patients in their homes. Responsibilities include patient assessment, family instruction, and reporting changes to the supervising nurse. CPR may be required; prior home health experience is preferred.
