Purpose and Scope The professional registered nurse Outpatient RN CAP1 is an entry‑level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. The role may include delegation of tasks to RN, LVN/LPN, and Patient Care Technicians. Principal Duties & Responsibilities Performs all essential functions under the direction of the Supervisor and guidance from the Clinical Educator, Preceptor, or collaborating Registered Nurses. Collects and analyzes patient data pre‑, during‑, and post‑hemodialysis treatment and documents results; adjusts treatment plans as indicated and notifies Team Leader, Charge Nurse, Supervisor, or Physician as needed. Assesses, collaborates, and documents patient/family learning needs to provide education. Directs and provides, in collaboration with patient‑care staff, safe and effective hemodialysis therapy. Administers medications as prescribed or per approved algorithms; documents justification and effectiveness. Initiates or assists with emergency response measures. Serves as a resource, participates in staff training and new‑staff orientation. Ensures correct laboratory collection, processing, and shipping; reschedules missed or insufficient collections. Documents and updates nursing assessment and plan of care in collaboration with the Interdisciplinary Team. Ensures patient awareness of transplant and treatment options. Completes CAP requirements to advance. Performs additional duties as assigned by Supervisor. Physical Demands & Working Conditions Direct patient care involving heavy lifting, moving patients, and assistance with ambulation; equipment or coworkers may assist. Requires frequent, prolonged standing and ability to bend over. May need to move up to 200lb. machines and lift up to 30lb. chemical solutions. Work environment: health‑care facility with controlled air temperature, moderate noise levels. Subject to exposure to infectious and contagious materials. Daily work includes desk, computer, patient interaction, facility/hospital staff and physician interaction. May require travel to training sites or other facilities. May provide essential functions at other locations with same physical demands. Supervision Assigns oversight of Patient Care Technicians/LPNs/LVNs/RNs as Team Leader or designated Nurse in Charge after meeting: Successful completion of all FKC education and training requirements for new employees. Minimum of 9 months RN experience. Minimum of 3 months chronic/acute hemodialysis RN experience. Education & Licensure Graduate of an accredited School of Nursing. Current appropriate state licensure. Current or completed CPR BLS Certification. Meets state practice requirements where employed. Experience & Required Skills Entry level for RNs with less than 2 years nephrology nursing experience. Chronic/acute hemodialysis experience preferred.
