Position Overview

BrightSpring Health Services is seeking a Physical Therapist for a Home Health assignment in Ashland, KY. This position offers the opportunity to provide one-on-one patient care in the home setting while working independently throughout the community. The Physical Therapist will evaluate patients, develop individualized treatment plans, coordinate care with the interdisciplinary team, and help patients improve mobility, function, and independence.

Responsibilities

• Complete evaluations, reassessments, and discharge planning
• Develop and implement individualized treatment plans
• Provide skilled physical therapy services in the home environment
• Collaborate with physicians, nurses, caregivers, and therapy staff
• Complete timely and accurate documentation
• Educate patients and families regarding safety, mobility, and home exercise programs
• Participate in interdisciplinary care planning

Requirements

• Active Kentucky Physical Therapist License
• Current BLS Certification
• Home Health experience preferred
• OASIS experience preferred
• Valid driver's license and reliable transportation
• Strong communication and organizational skills
