A full-time Medication Therapy Management (MTM) Pharmacist position is available for a 13-week contract-to-hire opportunity emphasizing specialty MTM services. This role offers the flexibility to work onsite or remotely and requires a current Louisiana Pharmacist license along with licensure in multiple states—including at least five of the following: Louisiana, DC, North Dakota, New Hampshire, New Jersey, South Carolina, South Dakota, Utah, and Wyoming. Candidates must have prior retail or hospital pharmacy experience to succeed in this role.

Key Responsibilities:

• Prepare and dispense medication orders per physician requests, adhering to established policies and protocols
• Interpret prescriptions accurately, maintaining comprehensive patient medication profiles
• Dispense pharmaceuticals, including controlled substances, ensuring compliance with legal regulations
• Monitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness
• Detect and report adverse drug reactions promptly
• Provide clinical consultation to practitioners and suggest cost-effective therapeutic alternatives
• Document all clinical activities and interventions completely and accurately
• Participate in quality improvement and medication use review activities within the department
• Supervise pharmacy support personnel, verify daily technician activities, and assist in performance appraisals
• Conduct medication storage area inspections, ensuring removal of outdated or unusable drugs
• Handle inquiries professionally via phone and service windows

Qualifications and Desired Experience:

• Current pharmacist license in Louisiana and licensure in a minimum of five additional states from the specified list
• Prior experience in retail or hospital pharmacy settings
• Ability to work independently with minimal supervision
• Strong organizational skills and attention to detail
• Commitment to patient safety and adherence to pharmacy regulations
