Job Summary:

The Licensed Vocational Nurse (LPN) with a Hawaii license will provide skilled nursing care in a long-term care setting. Responsibilities include medication administration, skin treatments, patient assessments, assistance with activities of daily living (ADL), and specialized care such as nebulized treatments, wound vac, CPAP, and gastrostomy tube management. The role requires collaboration with the healthcare team to ensure quality patient care in a skilled nursing facility. Responsibilities:

• Administer medications and perform skin treatments as prescribed.
• Conduct patient assessments and monitor health status.
• Assist patients with activities of daily living (ADL).
• Provide specialized treatments including nebulized therapy, wound vac care, CPAP care, and gastrostomy tube care.
• Document patient care and report changes to the interdisciplinary team.
• Collaborate with healthcare providers to develop and implement care plans.

Required Skills & Certifications:

• Active Licensed Vocational Nurse (LPN) license in the state of Hawaii.
• Proof of flu vaccination or approved medical/religious exemption prior to contract start.
• Experience in medication administration and patient assessment.
• Basic knowledge of wound care, respiratory therapy, and gastrostomy tube management.
• Ability to assist with ADLs and perform nursing treatments.

Preferred Skills & Certifications:

• Experience in long-term care or skilled nursing facility settings.
• Prior experience with wound vac and CPAP care.
• Strong communication and interdisciplinary collaboration skills.

Special Considerations:

• Must have Hawaii LPN license by start date.
• Flu vaccination is mandatory unless an approved exemption is provided.
• Work includes weekends, holidays, and potential overtime.
• May be required to rotate shifts and work varied 8-hour shifts.

Scheduling:

• Varied 8-hour shifts including Days (07:30 15:30), Evenings (15:00 23:30), and Nights (23:00 07:30).
• Shift assignments include weekends, holidays, and overtime on a rotating basis.
• No guarantee of specific hours or shift preferences.
