This is a full-time, on-site, patient-facing role. You report to the Market Chief Medical Officer (CMO), or Associate Medical Director (AMD), and are responsible for both direct patient care and clinical leadership for the Anderson, SC clinic.About the RoleYou will provide primary care for senior patients in a value-based care model while leading a team of clinicians to lead quality, outcomes, and patient experience. You will balance patient care with leadership responsibilities, supporting clinicians, improving performance, and advancing a patient-centered model of care.What You'll DoClinical CareYou will provide primary care for senior patients, including those with complex and chronic conditionsYou will see patients and manage ongoing care, typically spending 50–80% of your time in direct patient careYou will complete accurate documentation to support care and outcomesYou will manage a clinical inbox, including labs, messages, and follow-upsYou will coordinate care with specialists and interdisciplinary care teamsYou will apply Medicare risk adjustment principles through accurate condition documentationClinical LeadershipYou will lead a team of Physicians, Nurse Practitioners, and Physician AssistantsYou will coach clinicians using data, performance dashboards, and patient outcomesYou will participate in hiring, onboarding, and retention of clinical staffYou will provide performance feedback, complete evaluations, and support professional growthYou will foster a culture of collaboration, accountability, and continuous improvementQuality & PerformanceYou will improve performance in patient experience, quality, and clinical outcomesYou will monitor and improve important metrics such as:Net Promoter Score (NPS)HEDIS and Medicare Stars measuresTotal cost of care and avoidable utilizationYou will conduct chart reviews to identify care opportunities and guide improvementOperational Partnership (Dyad Model)You will partner with operational leaders to align on clinical and business goalsYou will collaborate on provider schedules, patient access, and care delivery strategiesYou will support performance management, team engagement, and clinic operationsYou will contribute to consistent communication and aligned decision-making across teamsWhat You Can ExpectYou will work in a value-based model focused on outcomes—not volumeYou will balance direct patient care with meaningful leadership impactYou will lead within a team-based care environment focused on whole-person careYou will mentor clinicians and shape clinical performanceYou will benefit from a supportive culture with PTO, CME, and manageable call expectationsRequired QualificationsMD or DO from an accredited programBoard Certified in Family Medicine, Internal Medicine, or GeriatricsActive, unrestricted medical license (or ability to obtain) in the state of practice8+ years of clinical experience2+ years of leadership or project leadership experiencePreferred QualificationsExperience managing Medicare Advantage patient panelsExperience in value-based care or coordinated care environmentsExperience supervising Advanced Practice Providers (NPs/PAs)Active, unrestricted DEA licenseMedicare and Medicaid provider enrollmentCompensation and BenefitsYou will receive a compensation package that may include:Sign-on bonus and relocation support (if applicable)Medical, dental, and vision insurancePaid time off (PTO) and CME support401(k) with employer matchScheduled Weekly Hours: 40Pay Range: $292,000 - $341,500 per yearThis job is eligible for a bonus incentive plan.
