Description We are seeking a licensed and dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing team. The provider will travel to skilled nursing facilities across Connecticut to perform monthly patient visits. This is an excellent opportunity for a motivated clinician who enjoys working independently, making a direct impact on patient care, and maintaining flexibility in their schedule.
