Family Practice/Primary Care Nurse Practitioner

We are working on an exciting new opportunity with a growing healthcare company that is seeking Nurse Practitioners or Physician Assistants in Cedar Rapids, IA to work visiting patients at home and virtually. -Hybrid, full time schedule: Monday through Friday 8:30 - 5:00 pm
-Home care or hospice experience preferred
-Chronic disease management experience preferred
-Must be willing to travel up to one hour from home
-Must be able to visit patients in their homes up to 4 days/week, 1 day remote
