Hospice Registered Nurse

Provide skilled nursing care and case management for hospice patients in their homes or at hospice centers. Ensure the delivery of comprehensive, compassionate care by coordinating services and facilitating positive clinical and quality-of-life outcomes.

Responsibilities:

• Deliver skilled nursing assessments and interventions based on physician-authorized care plans.
• Monitor, evaluate, and adjust care strategies according to patients' physical and psychosocial conditions.
• Supervise and collaborate with Home Health/Hospice Aides and CNAs to ensure plan adherence.
• Facilitate communication among interdisciplinary team members, community agencies, and family caregivers.
• Educate patients and families on symptom management, health maintenance, and available resources.
• Maintain detailed, compliant documentation in accordance with agency standards and regulatory requirements.

Qualifications:

• Possess a current RN license in Montana.
• Hold a valid driver's license, auto insurance, and reliable transportation.
• Bring at least 2 years of recent experience in acute, home care, or hospice nursing.
• Demonstrate clinical proficiency in venipuncture, wound care, catheterization, and parenteral therapy preferred.
• Exhibit strong assessment, communication, and team collaboration skills.
