Care Manager (Community Outreach and Care Coordination)

Care Manager (Community Outreach and Care Coordination) Job Location: Albany, NY, US Job Type: Full Time (Non-exempt) Schedule: Monday - Friday; 9am-5pm with flexibility Pay Range: $22.06 – $29.69 per hour Local travel required; a NYS Driver’s License and personal vehicle necessary. Position Overview Following the Children’s Health Homes Care Management program guidelines, the Health Homes Care Manager provides comprehensive care planning, ongoing care coordination, and community linkage to Medicaid-eligible children with complex physical and behavioral health conditions and their families. The aim is to improve overall health and well-being and foster independence. Duties & Responsibilities Uses a strength-based and client-centered approach to build rapport and establish a therapeutic relationship with the child and care-givers. Conducts comprehensive assessments of children’s healthcare needs and families’ social determinants of health using the designated health assessment tool and the NYS CANS assessment. Coordinates interdisciplinary team meetings with health care providers, social support providers, and family supports to develop the child’s Care Plan. Develops a Safety and Crisis Plan with input from the child and family. Performs tasks outlined in the child’s Care Plan, including referrals, monitoring access to providers, linkage to support services, coordination among the care team, and advocacy. Maintains the Care Plan and monitors completion of Care Plan goals. Provides transitional care during admissions and discharges from health care facilities. Documents delivery of billable health home core services and other case-related contacts in the EHR. Ensures required billable service and face-to-face contact benchmarks are met each month. Complies with deadlines for assessments, Care Plan development, and interdisciplinary meetings. Conducts comprehensive discharge planning for exiting children. Participates in monthly Quality Assurance meetings. Completes all mandated trainings set by DOH, contracted Health Homes, and MCOs. Assists with distribution and collection of bi-annual satisfaction surveys. Performs additional duties as assigned by the Supervisor. Position Specifications Bachelor’s degree in psychology, sociology, counseling, or other human services-related field required. Minimum 2 years of experience in child welfare, mental health, substance abuse, or physical health services required. Proficiency in Microsoft Office Suite, Google Workspace Suite, and secure Internet practices. Experience with AWARDS / NYS Connections considered a plus. Ability to pass certification as a CANS assessor. Regular and substantial unsupervised contact with clients; fingerprint required for criminal history check. Current Clean & Valid New York State Driver’s License; safe and reliable vehicle required. Must meet insurance policy requirements for authorized drivers (minimum age 21, maintain insurability). Strong attention to detail, excellent writing aptitude, ability to exercise ethical judgment and discretion handling sensitive data.
