Nurse Practitioner/Physician Assistant - Full Time PACE Central Michigan is preparing for growth with our new Houghton Lake expansion and continued service in Mt. Pleasant. We are seeking a Per Diem/ Part Time Physician Assistant or Nurse Practitioner to join our interdisciplinary team. This is a new position, forecasted as we expand over the next year, and offers the opportunity to work across both the Houghton Lake and Mt. Pleasant service areas. As our census grows, there is potential for this role to evolve into full-time employment The role provides flexibility, with shifts scheduled on an as-needed basis, and the chance to be part of a mission-driven program that helps seniors live safely at home and in their community. What is a PACE program? The Program of All-Inclusive Care for the Elderly (PACE) is a unique health care model designed to help seniors (participants) stay safely in their home for as long as possible. PACE coordinates and provides or facilitates all medical care for its participants. Services are delivered across various settings, including on site at the PACE center, in the community, and in participant homes. Why Work at PACE? At PACE, employees have the opportunity to see the full impact of their efforts. Staffing ratios are much lower, time spent with each participant is much greater, and team collaboration occurs much more frequently than in other care settings. PACE is a very fulfilling health care delivery model for its employees. Job Summary The Nurse Practitioner has the responsibility for providing direct and indirect care to the participants in the PACE Health Clinic and Day Center. Works under the direct supervision of the Medical Director/PCP as described in the Nurse Practitioner Collaborative Practice Agreement. Manages complex clinical situations for which he/she is responsible Duties and Responsibilities include but are not limited to: Performs health histories, physical exams and participant teaching. Orders, interprets and follows up on lab tests and results. Performs selected procedures and documents care in EMR. Initiates emergency measures as indicated. Recognizes abnormalities that justify immediate referral for intervention by others. Initiates, evaluates, reviews and changes orders for selected medications and diagnostic studies/treatments within Medical Director/PCP approved protocols. Assesses, initiates and coordinates participant care on a daily basis under the supervision of the Primary Care Physician. Makes referral to other physicians including specialists with Medical Director/PCP approval. Requests consultations from other services as needed in collaboration with Medical Director/PCP and/or established protocols. Acts as liaison with physicians, nurses and other healthcare professionals providing education, communication and support for healthcare staff, participants and families. Directs participant management in conjunction with Medical Director/PCP. Participates in the PACE provider call schedule. Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Assists the IDT in formulating a plan for the coordination of 24-hour care. Documents participant changes appropriately in the medical record and communicates participant changes to team members. Evaluates, treats and provides consultation with participants in their homes, assisted living facilities, nursing homes, and hospitals when medically indicated and as appropriate. Precepts any medical students and residents during their PACE rotation. Encourages all participants to have Advanced Directives and provides education as needed; may discuss the Do Not Resuscitate (DNR) option with the participant and/or Guardian/DPOA in collaboration with Medical Director/PCP Skills: Ability to learn and research; ability to solve problems and think analytically; must possess skills necessary to treat geriatric participants; ability to get along with others in a team environment. Ability to meet a standardized set of competencies as set forth by the Center for Medicare and Medicaid Services (CMS) for Programs of All-Inclusive Care for the Elderly (PACE). Have the ability to work independently with limited supervision for daily work priorities. Have a customer service attitude with a sense of urgency towards quality participant services. The ability to relate and work well with other employees of varying skills and educational levels. The ability to employ great collaborative skills both internally across departments and externally between other providers of relevant services. The ability to communicate effectively in all interactions with customers. The ability to build relationships with staff, other departments, participants, family, volunteers & visitors. Education and Experience: Master's degree in nursing from accredited Nurse Practitioner program. At least two (2) years nursing experience with Nurse Practitioner experience preferred and one (1) year experience with a frail or elderly population. Active RN licensure and specialty certification as a Nurse Practitioner in Michigan. Ability to meet a standardized set of competencies as set forth by the Center for Medicare and Medicaid Services (CMS) for Programs of All-Inclusive Care for the Elderly (PACE). Current CPR certification.
