Pediatric Care Transition RN Coach (Hybrid, FL)
Molina Healthcare
Molina Healthcare seeks a registered nurse in Florida for a hybrid role focused on care transitions and discharge planning. You will coordinate 30-day hospitalization-to-home transitions, support member needs, and work with hospital teams to reduce readmissions.
The role requires 2+ years in healthcare, RN license, valid driver's license, and ability to travel 40-50% locally. Proficiency with Care Transitions Intervention and MS Office is preferred.
