SubAcute Nurse Manager (Registered Nurse, RN or Licensed Practical Nurse, LPN)

We are seeking a dedicated and experienced Registered Nurse (RN) and/or Licensed Practical Nurse (LPN) to join our team as a Charge Nurse . In this role, you will support the Subacute Unit Manager in driving positive patient outcomes and delivering high-quality care. Must have Subacute Experience! Key Responsibilities Assist with oversight of patient care and outcomes Provide support with admissions and discharges Perform and manage wound care Monitor and support cardiac patients Communicate effectively with patients and families, including outreach as needed Qualifications Prior experience in subacute or skilled nursing preferred Strong clinical skills in wound care and cardiac care Experience with admissions, discharges, and care coordination Commitment to patient-centered care and teamwork Join a fantastic company and facility.
