The current need is every other Friday, with both visits totaling approximately 7-8 hours. The first two hours of each shift are paid at $50/hr , with $45/hr for all remaining hours. This nurse will also case manage both clients, including plan of care development , 60-day reassessments , and supply ordering . Mileage will be appropriately compensated. This is an excellent opportunity for a nurse who enjoys autonomy, relationship-based care, and a predictable, low‑frequency schedule. Job Responsibilities Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards. Collaborate with multidisciplinary team members, partnering pharmacies and providers to optimize patient care and outcomes. Educate patients and their families on health maintenance and disease prevention. Ensure accurate documentation and compliance with all regulatory guidelines. Requirements Active RN license in the state of Minnesota. Previous experience in a home health or acute care setting is advantageous. Exceptional interpersonal and communication skills.
