Key Responsibilities

• Perform physical therapy evaluations for a skilled nursing population
• Provide daily treatments to a high rehab-to-home patient population
• Complete discharge planning and documentation in accordance with facility protocols
• Maintain a productivity standard of 90%
• Adhere to dress code requirements (black scrubs) and onsite presence expectations

Required Qualifications

• Active Physical Therapist license in the state of New Mexico or eligibility to obtain
• Ability to start within 2–3 weeks
• Must be present onsite to receive expected weekly hours

Preferred Experience / Skills

• Prior experience in a skilled nursing facility (SNF) setting strongly preferred
• Experience with rehab-to-home patient populations
• Familiarity with SNF documentation and compliance standards
