Nurse Case Manager

American Senior Communities is seeking a Registered Nurse case manager to join our interdisciplinary team. The role supports resident care planning for a designated unit, in collaboration with the Unit Manager and ADNS, and adheres to federal/state regulations and facility policies. Ideal hours 10am-6pm with on-call every 7th weekend. Responsibilities include assessments, documentation of changes, and maintaining compassionate geriatric care. #J-18808-Ljbffr
