St. Luke's - 700 Lawn Avenue - Responsibilities: Assist in the delivery of healthcare support for practices within defined region(s).; Perform pre-visit planning by reviewing patient records and updating recent test results.; Complete pre-charting of value-based pieces up to five days prior to appointment.; Prepare patients for examinations including vital signs, vaccine administration within state guidelines, and updating records.; Travel throughout assigned geographic region to support SLPG practices and coordinate with Care Team.
