Unified Care NW is actively seeking a dedicated and experienced Licensed Independent Clinical Social Worker (LICSW) for a full-time position. This role is pivotal in providing mental health care and psychosocial support to patients of all ages in a compassionate and understanding manner. The clinician will primarily work remotely from the hospice center situated in Marysville, Washington, allowing for flexibility and focused care delivery.

Key Responsibilities

• Provide comprehensive clinical services to eligible individuals and families utilizing best practice standards, including evidence-based interventions tailored to meet the unique mental health needs of patients.
• Collaborate closely with schools, social service organizations, and healthcare providers to ensure a seamless continuum of care, advocating for patients and building effective support teams within the community.
• Utilize appropriate tools and methodologies to assess client needs, monitor progress, and adjust care plans as necessary, always placing client welfare at the forefront of all interventions.
• Engage in regular monitoring of cases related to Legal Representative Agreements (LRA), facilitating communication with court personnel and assisting with LRA compliance to ensure uninterrupted patient care.

Patient Population / Client Focus

The ideal candidate will work primarily with patients experiencing chronic health conditions, mental health challenges, and those who may be unhoused. They must possess a nuanced understanding of social dynamics affecting various demographics and be able to provide culturally competent care to all clients, fostering trust and support.

Qualifications

• Must hold a Bachelor's degree in Social Work or a closely related field and possess a current and valid LICSW license.
• A minimum of three years of clinical experience, particularly in behavioral health settings, is required.
• A strong foundation in the principles of integrated behavioral health assessments, with a willingness to undergo further training as needed to enhance service delivery.
• Knowledge and familiarity with Medicaid, Medicare, and private insurance processes is essential for effective patient advocacy and resource allocation.

Preferred Experience

• Experience working within primary care settings and familiarity with trauma-informed care practices.
• A proven track record of successfully supporting clients from diverse backgrounds, including those experiencing homelessness.
• Understanding of community resources and services that aid vulnerable populations is highly desirable.

Compensation and Benefits

We offer a competitive salary range of USD 100,000 to 105,000 annually, commensurate with experience. Our benefits package includes:
