About Innovative Healthcare- Metabolic Department:

We are a specialty diabetes practice. We do the work most primary care offices refer out: implantable continuous glucose monitoring, IV infusion therapy, insulin pump management, and ongoing chronic care for complex diabetic patients. Our patients are the ones who have been managed poorly somewhere else for years. We're growing fast, and we're building the operational backbone to grow without breaking.

The Role:

You own clinical operations for the center. Provider productivity, program compliance, clinical staff, supplies, and the SOPs that hold it all together.

This is not a policy seat. You'll set the standard, measure against it, and close the gap yourself when it opens.

What You'll Own:

Provider productivity:

• Establish visit and procedure benchmarks for all clinical providers.
• Monitor performance data weekly and identify outliers before they become trends.

CGM program:

• Own end-to-end operations for implantable CGM insertion and replacement, including scheduling, sensor inventory, manufacturer coordination, and procedure day flow.
• Track utilization and ensure every case aligns with payer coverage criteria and clinical protocol.
• Own documentation oversight: medical necessity, prior authorization support, and chart completeness before claims go out.

RPMCCM program:

• Own development of the Remote Patient Monitoring and Chronic Care Management program end to end.
• Track and report monthly on enrollment, active patient count, time-capture rate, revenue per enrolled patient, and utilization and efficiency by role.
• Manage device vendors, device inventory, connectivity failure rates, and patient device onboarding.
• Own patient churn. Find why enrolled patients go inactive and fix the enrollment or engagement gap causing it.

Infusion services:

• Manage infusion suite operations including scheduling, chair utilization, nursing coverage, and drug and supply management.
• Oversee pump refill and maintenance workflows and the documentation attached to them.
• Ensure infusion documentation and utilization hold up to payer scrutiny.

Clinical staff:

• Directly supervise clinical support staff including NPs, RNs, and medical assistants.
• Own scheduling, competency verification, and day-to-day accountability for the clinical floor.
• Run onboarding for new clinical providers and staff: orientation, workflow training, productivity expectations, and competency sign-off.

Referral and partner relationships:

• Serve as the primary operational contact for referring endocrinology, primary care, podiatry, and hospital partners.
• Manage referral intake flow and turnaround times so patients don't stall between referral and first visit.
• Identify and open new referral relationships alongside leadership.

Operations and compliance:

• Own supply chain for infusion supplies and clinical inventory, including par levels, ordering, and waste.
• Build and continuously improve standard operating procedures across clinical workflows.
• Maintain compliance with clinical standards and payer guidelines across all service lines.
• Partner with billing and credentialing on documentation quality, denials, and provider enrollment.

What You Need:

Required:

• Clinical license in good standing (RN, LPN or equivalent).
• 3+ years managing clinical staff in an outpatient, ambulatory, or specialty practice setting.
• Demonstrated ownership of provider productivity or clinical performance metrics, not just observation of them.
• Working knowledge of payer coverage criteria, medical necessity documentation, and prior authorization.
• Comfort in data. You should be able to open a productivity or utilization report and tell us what's wrong with it.

Preferred:

• Experience with RPM/CCM services
• Background in a high-growth or multi-site healthcare organization
• History of building SOPs and onboarding programs from scratch

Compensation and Benefits:

• Salary range: $80,000-$95,000 commensurate with experience.
