CenterWell Home Health is seeking an experienced LPN to provide one-on-one nursing care in patients' homes, under the supervision of an RN. You will implement care plans, educate families, document visits, and coordinate with the care team to support recovery. The role requires a current state nursing license, CPR certification, a valid driver's license with reliable transportation, and prior LPN/LVN experience in a clinical setting.
