You will: Conduct comprehensive admission assessments Ensure accurate medication reconciliation and care planning Coordinate with interdisciplinary teams to support smooth transitions Serve as a clinical leader and resident advocate Maintain compliance with all clinical and regulatory standards Your clinical judgment, attention to detail, and compassion will directly impact resident safety, comfort, and outcomes.
