• Provides direct training and support by guiding clients and encouraging while ensuring that the Individual Service Plan goals are being achieved.
• Follows and maintains all Company instructions, policies, and procedures
• Provides appropriate services and supports according to the needs, wishes, and established plan of each client.  Adheres to the individual standards for each client.
• Supports and respects client’s communication needs including consistent use, training, ongoing education, and practice of facilitated communication.  Consistently communicates with clients’, honoring client’s individual choice of communication.  Assists clients with all personal correspondence, opening and reading all mail and faxes in a timely manner. Assists clients in communicating by phone, email or in writing.  Answers phone messages and mail in a timely manner. Routes all pertinent information to the CSF (Community Services Facilitator)
• Ensures that the physical/safety, health, and support needs of the client are being met.  
• Under the supervision of the CSF, the PA will assist the client with medications and appointments in the community ( Please note that the CSF is in charge of clients’ medications and doctors’ appointments )
• Demonstrates awareness and understanding of client rights and abuse reporting procedures required by Title 17 and the Welfare and Institution code. 
• Assists client in gaining education of chemical safety.  Keeps medicines and toxic materials locked if or when client does not demonstrate skills is chemical safety.   Reports potential health problems immediately. 
• Provides pertinent input in developing goals/objectives for clients’ living situations, as necessary, including participation in person directed planning for each client.  Participate with client and their respective circle of support, in developing and supporting or implementing client Individual Service Plan & IPP and any other program or services plan.
• Attends quarterly and annual training
• Advocates for the clients whenever needed
