Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting to restore or maintain functional independence
• Develop and implement individualized occupational therapy treatment plans based on patient needs and goals
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary team members to coordinate comprehensive patient care
• May be required to float to a nearby affiliated senior living location as needed

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Illinois
• Graduate of an accredited Occupational Therapy program
• Ability to work day shift in a skilled nursing facility environment
• Willingness to float to nearby affiliated facility locations as assigned

Preferred Experience / Skills

• Prior experience in a skilled nursing or long-term care setting
• Familiarity with geriatric patient populations and related treatment approaches
• Experience with electronic documentation systems
