Key Responsibilities
- Evaluate and treat patients in a skilled nursing facility setting to restore or maintain functional independence
- Develop and implement individualized occupational therapy treatment plans based on patient needs and goals
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary team members to coordinate comprehensive patient care
- May be required to float to a nearby affiliated senior living location as needed
Required Qualifications
- Active Occupational Therapist (OT) license in the state of Illinois
- Graduate of an accredited Occupational Therapy program
- Ability to work day shift in a skilled nursing facility environment
- Willingness to float to nearby affiliated facility locations as assigned
Preferred Experience / Skills
- Prior experience in a skilled nursing or long-term care setting
- Familiarity with geriatric patient populations and related treatment approaches
- Experience with electronic documentation systems
