Licensed Practical NurseJCCA is seeking a qualified Licensed Practical Nurses to provide primary health care management for assigned cases collaborating with all members of health care and clinical team.Summary of Job Functions:Provide primary health care management for assigned cases.Maintain accurate and up-to-date Medication Administration RecordsActively communicate with community providers scheduling needed appointments and obtaining health related information for clientsCommunicate and coordinate with clinical team, foster and biological family/guardian about all medical conditions, appointments and/or treatments.Provide formal and informal training for foster and biological parents on specific health care needs of youth, including appropriate health practices and administration of medications.Qualifications:NYS Licensed Practice Nurse requiredPediatric and/or Medical/Surgical experienceExcellent organizational skills and follow up.Effective communication skills - both written and oral$30 -$35 per hour
