Registered Nurse Case Manager

Job Summary:
We seek a highly skilled and experienced Registered Nurse with at least 2 years of acute medical care management experience to perform care coordination and case management in an acute care facility. The Registered Nurse will provide skilled nursing care, patient assessments, medication administration, treatment planning, and collaborate with the interdisciplinary team. This role requires strong communication, travel experience, and the ability to float between facilities as needed.
Responsibilities:
We seek a highly skilled and experienced Registered Nurse with at least 2 years of acute medical care management experience to perform care coordination and case management in an acute care facility. The Registered Nurse will provide skilled nursing care, patient assessments, medication administration, treatment planning, and collaborate with the interdisciplinary team. This role requires strong communication, travel experience, and the ability to float between facilities as needed.
Responsibilities:
- Perform care coordination and case management for acute medical patients.
- Conduct patient assessments and develop treatment plans.
- Administer medications and provide skilled nursing care.
- Collaborate effectively with interdisciplinary healthcare teams.
- Float between the primary and Laguna facilities as required.
- Maintain patient ratios of approximately 1:25.
- Adhere to facility policies and procedures regarding patient care and documentation.
- Active California Registered Nurse license or pending license with licensure upon start date.
- Minimum of 2 years acute medical care management experience performing care coordination.
- Strong clinical assessment, medication administration, and treatment planning skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced acute care environment.
- Travel experience required.
- Experience working in acute care facilities with floating responsibilities.
- Business casual or turquoise scrub attire familiarity.
- Floating expectation to Laguna facility.
- Weekend work is variable; department operates 7 days per week.
- Days off may not be back-to-back but scheduling efforts will be made for consecutive days off.
- RTO restrictions: no more than 7 days.
- Patient ratio approximately 1:25.
- 5-day workweek with variable weekend requirements.
- Shift assignments may vary to cover department operations throughout the week.
- Start date: ASAP.
