Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)
Occupational Therapy Assistant Job Duties:
Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
Monitor and document patient progress with the use of innovative EMR software.
Occupational Therapy Assistants Employed by Flagstar Rehab Will Receive:

Occupational Therapy Assistant Requirements:
Current NYS OTA License and Registration
EMR Competence
State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation)
Valid Proof of Identity and Work Authorization
Malpractice/Liability Insurance (if applicable)
