The Medical Specialist 2 is a licensed, Board-Certified physician solely qualified to practice medicine in the state of New York. The Taconic district spans five counties including Putnam, Dutchess, Ulster, Columbia and Greene counties, and provides services to over 300 individuals with ID/DD, many of whom are advanced in age and have complex medical issues and physical challenges.

The Medical Specialist 2 physician will serve approximately 50% of the time within the Article 16 Clinic, as the Article 16 Clinic Medical Director, and the other 50% of the time, as the State Operations Residential physician. Medical Specialist 2 hired for this position would either be a Board-Certified Neurologist or a Board-Certified Internist.

The Medical Specialist 2 will be supervised by the Taconic DDSOO Region 4 Medical Director and will have a base work location at 76 Firemen's Way, Poughkeepsie N.Y.; additionally, the physician will be expected to rotate at least once per month, at the Boices Lane Article 16 Satellite Clinic site, Kingston N.Y., or occasionally at other certified satellite clinic sites, per the discretion of the Taconic DDSOO Medical Director.

Article 16 Clinic Medical Director Duties

The responsibility for the ongoing direction of clinical services provided by clinicians of the Article 16 Clinic.

The Clinic Medical Director is licensed to practice medicine in New York State and is designated as responsible for maintaining the general health conditions and practices of the clinic department program.

The Article 16 Clinic Medical Director is responsible for approving all clinicians' Clinical Treatment Plans within the agency's Electronic Medical Record (EHR) sent by the Clinic Treatment Coordinator.

The Article 16 Clinic Medical Director is responsible for reviewing and approving all requests for Clinic Treatment Services within the agency's Electronic Medical Record (EHR) sent by the Clinic Treatment Coordinator.

The Article 16 Clinic Medical Director is responsible for conducting, completing, and approving all Physician "MD" Annual Assessments within the agency's Electronic Medical Record (EHR).

The Article 16 Clinic Medical Director participates in policy development and implementation for the Article 16 Clinic, as appropriate and applicable.

State Operations Duties

On Call after hours responsibilities as indicated by Taconic's Medical Director.

Reviews patients for MOLST & OPWDD Checklist process Develops, implements, and reviews medical training intervention programs for staff, in consultation with nursing.

Conducts physical examinations; orders, performs, and interprets diagnostic tests and procedures; determines and/or confirms diagnoses; and treats a variety of specialized medical conditions.

Prescribe and/or administer treatments, medications, therapies, vaccinations, and other specialized medical care to treat or prevent illness, disease, or injury; conduct follow-up examinations and monitor individuals' conditions, modifying treatment as needed.

May collaborate with other health care professionals and disciplines to achieve treatment goals and objectives and ensure continuity of care.

Maintains medical documentation consistent with OPWDD agency, New York State, and federal requirements.

Functions as a member of an interdisciplinary treatment team, attends meetings, and serves on various committees and workgroups to improve individual services and outcomes.

Will be expected to maintain a good time and attendance record, complete all mandatory trainings, and perform tasks as assigned by supervisor.
