Certification Details
- Position: Respiratory Therapist
- Job involves working night shifts with 3x12 hour night shifts.
- No on-call duties.
- Work schedule includes every other weekend and every other holiday off.
Job Details
- Must have BLS certification (American Heart Association).
- Must have ACLS certification (American Heart Association).
- Must have RRT or CRT certification.
- License/Certificate verification required, must be dated within 30 days of start.
- Fit Test required.
- Trinity Specialty Specific Competency Testing must be completed within a year.
- COVID-19 Vaccine proof required (including booster or declination/exemption).
- MMR vaccination proof or IgG titer required.
- Hepatitis B vaccination series required (proof of 1st shot prior to start).
- 2 Step PPD OR IGRA (Quantiferon/Tspot) required.
- Varicella vaccination proof or IgG titer required.
- Physical/Health Statement from exam within last year required.
- Trinity 10 Panel Drug Screen required within 30 days of start.
- Trinity Code of Conduct Form required.
- Trinity I9 form required.
- Trinity Confidentiality and Security Agreement required.
- Trinity Integrity and Audit Services Form required.
- Trinity Non Employee Upload Form required.
- Trinity Profile (Resume, Skills Checklist, 2 Professional References) required.
- Trinity Conflict Of Interest Disclosure Form required.
- Trinity Criminal Background Check required (within 30 days).
- Trinity Education Verification required.
- Trinity EPLS report required (within 30 days).
- Trinity GSA/OIG report required (within 30 days).
- Trinity Joint Commission Testing (CORE Exam within a year) required.
- Trinity NSO report required (within 30 days).
- Trinity System Access Form must be completed and uploaded one week before start.
- Trinity Skills Checklist required for clinical contractors.
- Tdap vaccine required or declination if no pediatric interaction.
- Trinity Child and Adult Dependent Abuse Training (Iowa only).
- Trinity IA Sing Report (Iowa Criminal History and Abuse Registries check) required within 30 days prior to start.
- Trinity OFAC screening required within 30 days of start.
Job Requirements
- Shift: 18:30 - 07:00 (Night shift)
- 3 shifts per week, each 12 hours long
