Job Title: Hospice Registered Nurse (RN) Profession: Nursing Specialty: Hospice RN Duration: 13 weeks Shift: Days Hours per Shift: 32 Experience: Minimum of 3 years License: Current Registered Nurse (RN) license in the state of Utah or compact state RN license Certifications: Current BLS Certification Must-Have: - Strong clinical assessment skills - Excellent communication, interpersonal, and organizational skills - Compassionate demeanor and ability to support patients and families during difficult times - Valid driver’s license and reliable transportation required for home visits Description: We are seeking a dedicated and compassionate Hospice Registered Nurse (RN) to join our interdisciplinary team. The RN will provide skilled nursing care and support to terminally ill patients and their families in accordance with the patient’s plan of care and hospice philosophy. Key Responsibilities include: Performing comprehensive assessments of patients' needs and coordinating individualized care plans. Providing pain and symptom management consistent with the hospice plan of care. Educating and supporting patients and families regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy. Collaborating with the interdisciplinary team including physicians, social workers, chaplains, and volunteers. Participating in on-call rotations. Maintaining accurate and timely documentation in compliance with state, federal, and organizational policies. The position requires the ability to respond within 45 minutes to patient needs when on call. On-call shifts will include 1-3 shifts per week, including 3 weekends during the contract. Working on holidays is also required. Experience with EMR systems is preferred.
