Home Health Physical Therapist (PT) – Flexible Schedule | West Palm Beach, FL Concierge Home Care is hiring a Home Health Physical Therapist in the West Palm Beach area. This role offers flexible schedule, meaningful patient relationships, and income based on productivity. Coverage Area: West Palm Beach, FL Compensation & Earning Potential Pay‑per‑visit model: $70–$90 per visit + Medicare differential pay Average full‑time earnings: $95,000 – $120,000+ Top performers earn $110K+ annually Quarterly bonus opportunities Mileage reimbursement + data plan stipend Benefits & Perks Health insurance (starts after 30 days) PTO begins accruing on day one Flexible employment options: Full‑time, PRN, or 1099 Supportive leadership and team culture Qualifications Active, unencumbered Florida Physical Therapist license Valid driver’s license, reliable transportation, and auto insurance Responsibilities Evaluate patients and develop individualized treatment plans Provide therapeutic exercises and mobility training Educate patients and caregivers on safety and recovery Collaborate with physicians and interdisciplinary teams Supervise Physical Therapist Assistants (PTAs) Complete documentation using streamlined technology Employment is contingent upon successful completion of an AHCA Level 2 background screening.
