Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (07:30 - 16:00) Experience: Minimum of 1 year of acute care experience License: Active RN license required Certifications: BLS preferred, ACM certification preferred Must-Have: Epic experience required Acute care experience required Description: The role requires overseeing patient care coordination and discharge planning. Candidates should possess strong assessment and planning skills. Experience in utilizing Milliman Guidelines is preferred. The position involves collaboration with interdisciplinary teams to ensure comprehensive patient care. Candidates must be available for weekend shifts every other week. No on-call duties are required. Local candidates are not accepted; candidates must reside more than 50 miles from the facility. The facility maintains a high daily census and serves a diverse patient age group, including neonates, children, adults, and geriatrics. Behavioral health experience does not apply to this role. Strong communication skills and adherence to HIPAA guidelines are essential. A maximum of one time-off request of five days or less is permitted during the assignment. Availability to work on two out of three major holidays (Thanksgiving, Christmas, and New Year) is required. Candidates will float within the unit to meet departmental needs when necessary. Physical and digital training modules are required and will be self-reported. Completion of the modules is recorded in transcripts.
