You will be directly working with patients, following a one-on-one care plan in patients' homes. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Responsibilities Maintain records of patient care, conditions, progress or problems to report and discuss observations with supervisor Assist client with full activities of daily living (ADLs) Assist with cooking, medication reminders, extensive transfer support (bed, chair, mobility transitions, if needed) Assist with ambulation (wheelchair support) Provide skilled services (wound care) if needed Hoyer Lift experience required Must have experience working with people with Disabilities/Amputees Must have a car Qualifications Experience as a caregiver, certified home health aide, or CNA Ability to work with one-on-one care plans in home settings Strong communication skills (verbal and written) #J-18808-Ljbffr Freedom Home Care LLC
